Register with Us

  1. Provide Documentation of Disability: To register with the Office of Disability Services for academic accommodations, you will need to submit the Student Intake Form and Documentation of Disability. You may choose to complete the optional Priority Registration Form if you would like to receive priority registration. You may provide documentation of disability in several ways, including, but not limited to: 
    • Disability Services Verification Form
    • IEP or 504 Plan
    • Neuropsychological Evaluation
    • A letter from a licensed medical provider, written on official letterhead which includes the information requested in the Verification Form  This is not an exhaustive list and ODS may request additional documentation in order to illustrate a connection between the impact of the disability, the described barrier, and the requested accommodation. During the exploration process, ODS will evaluate the unique attributes and requirements of the course, program, or activity. Course modifications, auxiliary aids, or services that are ineffective or constitute a fundamental alteration will not be reasonable and therefore will not meet the ADA and Section 504’s minimal standards.
    • Note: To avoid any conflict of interest, the Office of Disability Services will not accept documentation provided by family members, close relatives, and/or people not serving in official capacity with the student.
  2. Submit the documentation to stuaccess@ltu.edu and schedule an intake meeting with the Disability Services Advisor.  At the intake appointment, you will discuss your request with the Disability Services Advisor. From here, appropriate and reasonable accommodations will be discussed and agreed upon by both parties. Accommodation memos will then be emailed to each of your professors detailing the class and/or examination accommodations you are approved for.
    • *As per our confidentiality policy, no information regarding the student’s specific disability will be disclosed on the accommodation memos unless specified by the student
After Registering:
  • Schedule follow-up appointments: After your intake appointment, follow-up appointments can be scheduled at any time with the Disability Services Office.
  • Changes: You are tasked with notifying the Disability Services Office if circumstances change or additional needs arise. This can include some of the following:
    • The dropping/adding of classes
    • The decision to take a semester off
    • Additional accommodations are needed

» Eligibility

The Office of Disability Services at Lawrence Technological University determines eligibility on an individual basis. In order to be eligible, the student must provide disability-related documentation from the appropriate licensed and qualified professional to verify a student as having a disability and to determine reasonable accommodations.

For your convenience, we have provided a printable Disability Services Verification Form. Please be aware that if the initial documentation is incomplete, or not sufficient to determine the needed accommodations, the Office of Disability Services has the discretion to require additional information or documentation.

Once adequate and appropriate documentation is received, the Office of Disability Services will certify that a student has a disability. The student’s documentation will remain confidential and be on file in the Office of Disability Services.

» Attending College

The transition from high school to college is a challenging, rewarding, and exciting process. But let us not forget that information is key to making this journey a success! Below are some quick bits of information that all high school students should understand prior to applying for postsecondary education. This information is very important for students with disabilities to know since there are similarities and differences between high school and college services and accommodations.

  • Colleges do not provide personal aides but they may assist in making arrangements.
  • Generally have extended time on quizzes and exams, but not unlimited time.
  • There is no extra time to complete homework assignments, projects, term papers, etc. (Time Management is Key!).
  • No modified assignments, such as as shortened length.
  • Confidentiality of education records (FERPA) – this does not allow parents to discuss information without a student release form.
  • The typical college schedule demands very different time management skills compared to high school.
  • No resource room or special education classes.
  • Colleges rely less on human readers/scribes and more on technology (However, this does not mean that such accommodations are not available).
  • Colleges rely on diagnostic information to assist students but do not use the IEP process.

Use Your Cell Phone as a Document Camera in Zoom

  • What you will need to have and do
  • Download the mobile Zoom app (either App Store or Google Play)
  • Have your phone plugged in
  • Set up video stand phone holder

From Computer

Log in and start your Zoom session with participants

From Phone

  • Start the Zoom session on your phone app (suggest setting your phone to “Do not disturb” since your phone screen will be seen in Zoom)
  • Type in the Meeting ID and Join
  • Do not use phone audio option to avoid feedback
  • Select “share content” and “screen” to share your cell phone’s screen in your Zoom session
  • Select “start broadcast” from Zoom app. The home screen of your cell phone is now being shared with your participants.

To use your cell phone as a makeshift document camera

  • Open (swipe to switch apps) and select the camera app on your phone
  • Start in photo mode and aim the camera at whatever materials you would like to share
  • This is where you will have to position what you want to share to get the best view – but you will see ‘how you are doing’ in the main Zoom session.
