The Office of International Programs now uses paperless forms. We will no longer be accepting the old, paper versions. This website has direct links to all the forms that academic advisors should complete. Students should NOT complete any forms on this page. 

Academic Training Recommendation

Use this form to recommend AT employment opportunity for a J1 student. The Academic Training guide that outlines the AT policy is available for review  here . Students will have to submit additional documentation via email.

CPT Recommendation

Use this form to recommend a CPT employment opportunity for a student. The CPT guide that outlines the CPT policy is available for review  here . Students will have to submit additional documentation via email. 

OPT Graduation Verification

Use this form to verify a student’s expected graduation when they are in their last semester. This is the same form as the RCL verification, and can be used for both purposes.

Program Extension Recommendation

Use this form when a student needs an extension of their program of study. Students are responsible for knowing the program end date on their I-20 or DS-2019 and may need additional time to complete their degree.

Reduced Course Load (RCL) – Last Semester

Use this form to verify a student’s expected graduation and reduced course load when they are in their last semester. Students cannot be in only distance learning (i.e. online classes, hybrid classes, etc.) during their last semester. This is the same form as the OPT graduation verification, and can be used for both purposes.

Thesis/Doctoral Enrollment Verification

Use this form to verify a student’s enrollment is considered full time when at the doctoral level since enrollment requirements vary by semester.

Use Your Cell Phone as a Document Camera in Zoom

  • What you will need to have and do
  • Download the mobile Zoom app (either App Store or Google Play)
  • Have your phone plugged in
  • Set up video stand phone holder

From Computer

Log in and start your Zoom session with participants

From Phone

  • Start the Zoom session on your phone app (suggest setting your phone to “Do not disturb” since your phone screen will be seen in Zoom)
  • Type in the Meeting ID and Join
  • Do not use phone audio option to avoid feedback
  • Select “share content” and “screen” to share your cell phone’s screen in your Zoom session
  • Select “start broadcast” from Zoom app. The home screen of your cell phone is now being shared with your participants.

To use your cell phone as a makeshift document camera

  • Open (swipe to switch apps) and select the camera app on your phone
  • Start in photo mode and aim the camera at whatever materials you would like to share
  • This is where you will have to position what you want to share to get the best view – but you will see ‘how you are doing’ in the main Zoom session.
