This form should be filled out by all students interested or already in a fraternity or sorority on campus.

Greek Organization Roster Form
This form must be filled out every semseter and submitted to the Office of Student Engagement with all current organization members.

Greek GPA Request Form
This form must be filled out if the organization wishes to obtain the GPA of any of its members.  In order to obtain a member's GPA, said memeber must have already filled out the Greek Membership Form.
