Departure Notice

If you are leaving Lawrence Technological University, the Office of International Student Affairs, needs to know. Please complete the Departure Notice and submit it to the Office of International Student Affairs.  

This form not only enables the University to comply with its federal requirements, but also guarantees that mail, which may arrive at the Office of International Student Affairs after your departure, is forwarded to the correct address. Thank you for your help in this critical procedure.
